Challenge

Chemotherapy, while lifesaving, is a demanding treatment that requires careful monitoring of patient tolerance. With as many as one-third of patients arriving at the hospital, only to be sent home because of their inability to withstand treatment due to its side effects, this healthcare client suffered inefficiencies, wasted resources, and patient discomfort. Ensuring effective chemotherapy care coordination was a significant challenge for the client, and even more so for the cancer patients.

Solution

To address these challenges, we deployed a team of skilled telehealth nurses to engage with patients a few days before their chemotherapy appointments. During these pre-treatment interactions, nurses assessed the patient’s side effects and overall health, identifying potential issues before arrival.

A symptom analysis flagged any concerning patient responses during assessments. This data was immediately shared with the patient’s medical team, enabling them to adapt care plans or postpone treatments based on clinical and biological tolerances. By understanding patient needs ahead of time, the medical team could anticipate potential issues and provide supportive, preemptive care solutions to improve treatment tolerance.

Through this care coordination, the provider also avoided the complexity and cost of preparing chemotherapy in advance, with the discarded packs going to waste, making the process more efficient and less costly.

Our partnership successfully addressed a key challenge in oncology care, resulting in a system that prioritized patient welfare and operational success.

The results from this successful care coordination case were published by The Oncologist, European Journal of Oncology, the American Society of Clinical Oncology (ASCO), and the European Society for Medical Oncology (ESMO).
